If a gun fails to eject the first place I look is the ejector. That's the ejectors job hence it's name. The ejector pushes the spent case against the extractor and the tension created between the two will eject the spent case out of the gun. There are a few different types of ejectors. Some are spring and plunger in the bolt face others are free standing attached to the frame or action.
Another issue I look at in semi auto guns is whether or not the slide or bolt is cycling all the way. This will cause the spent case to not come into contact with the ejector and in the forward movement of the slide or bolt catch another round and cause a jam. If the gun is not cycling all the way then there are different issues at hand.
